A fiancee visa will allow you to remain in the United States for 90 days after the Fiancee Visa is approved. You will not be able to enter the US while your Fiancee Visa is pending. During that 90 days you have to get married, or else you will have to return to your home country no matter what the circumstance was. I believe the wait time for obtaining a fiancee visa is anywhere from 6-12 months after filing.
If you get married, it will depend on where you file you paper work from in order to answer you questions.
If you file for a green card from you home country, you will not be allowed to enter the US until you're application is approved (which I've heard can take up to 2 years or more). Your spouse will have to fill out an I-130 to sponsor you, and then you'll have to wait for that to be approved before you can come to the US and file for Permanent Residency.
If you file for a green card when you are in the USA (and married), then you can file your I-130 and Permanent Residency application at the same time. Once you submit your I-130 and I-485, you will not be allowed to leave the US until you get your Green Card unless you file for advanced travel. Even if your advanced travel form is approved, it is not a guarantee that you will be allowed to return to the US after leaving though.
In order to work in the US while you are waiting for you Green Card to be approved, you will have to file for a Work Authorization Card (WAC). After you get the WAC, you can file for a Social Security Card/Number and then you will be able to work after all that is approved. You will have to renew your WAC every year (including paying the fees) until your Green Card is approved. I would estimate the WAC approval time and SSN number approval at around 6 months or so.

As the first person said, it's better to get a fiance visa (K-1) to come to the US and then get married within the next 90 days. After that you can apply for a green card based on marriage.

It took right under two years for my husband to get his green card. It all really depends on what country you are coming from.
You can get a fiance visa. It's good for 90 days and you must get married during those 90 days. You can file for updated status as soon as you get married. It seems to move along a bit quicker this way, from what I've heard.
